Low Level Laser Therapy (LLLT) or ‘Cold Laser’ therapy involves placing a laser device close to or directly against the skin. Laser light is delivered directly over, and adjacent to the area of complaint, resulting in a range of beneficial physiological responses. LLLT uses low energy densities to stimulate the healing and repair of tissue. Unlike high powered lasers, LLLT does not burn, damage or cut tissue. As well as stimulating the body’s natural healing processes, LLLT also reduces pain and inflammation, enhances cellular metabolism, improves musculoskeletal function and modulates immune activity.

The lasers used at Ultima Healthcare are the most technologically advanced devices currently available. Many clinics use LEDs (light emitting diodes) which are not the same as lasers and have limited therapeutic capabilities. LEDs emit visible red light which is non coherent. Non coherent light is unable to effectively penetrate tissue, alleviate pain or stimulate healing.

LLLT uses coherent laser light which is measured in wavelengths known as nano-meters (nm). The wavelengths typically used in laser therapy range from the visible light spectrum (650 nm) up to the infra-red light spectrum (904 nm). The type of condition being treated will determine what wavelength will be used. For example, dermatological conditions respond better to visible red light (650 nm) whereas musculoskeletal conditions respond better to infra-red light (904 nm). This is because lower wavelengths (650 nm) only possess the capacity to penetrate superficial layers of skin, whereas higher wavelengths (904 nm) can penetrate deep in to tissue.

Low Level Laser Therapy (LLLT) has a wide range of beneficial effects at both the cellular and molecular levels. Below are some proposed mechanisms of how LLLT works.
  • Mitochondria within cells absorb laser light (specifically the chromophore known as cytochrome C oxidase), resulting in a significant increase in energy production. This increase in energy production allows damaged tissue to regenerate and heal faster.
  • Immune cells such as macrophages, fibroblasts and lymphocytes are stimulated by laser light to repair damaged tissue and modulate the inflammatory response.
  • Endorphins and neuropeptides (the body’s feel good chemicals) are released in response to LLLT which reduces pain and promotes healing.
  • LLLT may reduce pain by blocking pain signals travelling to the brain via the central nervous system.

A thorough case history and physical examination is undertaken at the beginning of the consultation to identify the best course of action to treat your condition. Depending on your condition, you will be asked to sit or lie down on the treatment table. You will also be provided safety glasses to wear during the treatment to protect your eyes. The laser device will then be placed on the skin, directly over and adjacent to the area of complaint. Patients undergoing LLLT do not typically feel anything during a treatment, however some patients report they feel a warm or slight tingling sensation when they laser is being applied

SCENAR therapy is regularly used in combination with LLLT for improved healing and pain relieving effects. SCENAR therapy is a form of biofeedback and feels like a mild vibrating sensation. The device is placed on the skin and moved in a ‘brushing motion’ over the area of complaint. The SCENAR device sends electrical impulses through the nervous system to further alleviate pain and improve function.

Low Level Laser Therapy has been used to treat patients from all over the world for over 30 years. LLLT has been evaluated for safety and efficacy in hundreds of clinical studies, with no expected long term adverse effects. Treatment with LLLT does no cause any pain, discomfort, burning, cutting or damage to tissue. Some patients may experience a very mild warming or tingling sensation during treatment. In the 12 - 24 hours after treatment, some patients may feel tired or notice that their pain has become more noticeable. This an expected and normal response that resolves after a good night sleep.

Initial consultations last 60 minutes as a thorough case history and physical examination needs to be undertaken to determine the best course of treatment.

Subsequent consultations last up to 30 minutes. The size of the area requiring treatment determines how long the laser needs to be applied. For example, treating a lower back will take longer to treat than a wrist.

Depending on the condition, the laser device is usually held over a single point for 30 – 180 seconds. A typical laser consultation requires anywhere from 10 – 20 points being treated.

Most acute conditions & injuries require 2 – 6 treatments. Acute conditions benefit from more frequent treatments and we recommend 3 – 4 days between visits. As the condition improves, the time between treatments can be extended to 1 week.

Chronic conditions require 6 – 12 treatments. We recommend 2 treatments in the first week to initiate the healing response and then 1 treatment per week thereafter, until the condition has resolved.

Every patient is an individual and therefore every patient responds differently. Some patients experience complete relief immediately after one treatment where as other patients require up to 12 treatments for their condition to be completely resolved.
